Developed in partnership with the Patient & Family Care Center at Michigan Medicine, the Stories for Care Toolkit was created to address a core challenge: moving beyond identifying problems to actively designing and testing solutions.

Grounded in real stories from patients, caregivers, and staff, the toolkit provides PFCC teams with a structured, repeatable process for surfacing unmet needs, co-creating interventions, and scaling what works. It includes hands-on methods, templates, and guidance to support discovery, design, and reflection—making human-centered innovation part of everyday clinical practice.
